Hospice care can dramatically and positively impact quality of life. The goal is to empower the individual by providing knowledge and information to facilitate decision-making. Our staff consists of experts on pain control, symptom management, and emotional and spiritual comfort. Furthermore, our staff is committed to the education of patients and caregivers.
Care is delivered by a team of professionals who address the patient’s needs on medical, physical, emotional, and spiritual levels. The team consists of the patient’s attending physician, hospice medical director, registered nurse, hospice aide, social worker, chaplain, and trained volunteers.
Anyone who has a life-limiting illness and a life expectancy of six months or less if the disease follows its normal course is eligible for hospice care. The patient or the patient’s representative chooses comfort care instead of curative or aggressive treatment, which is no longer effective or appropriate.
People who choose hospice are not giving up hope, but instead are redefining it. Though there may no longer be the possibility of curing their illness, they redirect their hope into mending and restoring relationships, spending quality time with those they love, and finding peace and comfort.
Help is only one phone call away. The attending physician, patient, family members, friends, clergy, social workers, hospital discharge planner, or any personal concerned with the patient’s wellbeing can make a referral. If you call us, we will contact the patient’s physician for approval. Services will be started immediately.
Admitting a patient to the hospice program is a simple process for the attending physician, discharge planner, or social worker. Just call the hospice and a nurse will visit the patient and family at home or in the hospital. Information about the program will be presented, and if the patient elects the program, the nurse arranges for start of services.
Visit our locations page to find a program near you. Call the location phone number 24-hours a day, 7-days a week. We'll help you learn about the process or schedule a no-obligation consultation.
Our admissions coordinator will promptly contact all responsible parties to plan consultations or patient assessments.
Our admission team works directly with families and healthcare professionals to begin care. We bring hospice care to where the patient calls home, whether in a private home, nursing home or long-term care community.
